What Your Membership Includes

One each month up to 60 mins & annual physical

In-person or by telehealth (at the physician’s discretion). Physical includes EKG.

Lab orders, result review, and referrals

When clinically appropriate. Testing is done at an outside lab and billed separately.

Preventive care, chronic-condition management, prescription refills between visits, medication and hormone-related care, and a plan built around you.

This membership is not health insurance. Insurance may still be needed for medications, hospitalizations, imaging, laboratory testing, referrals, and other services.
